What is erectile dysfunction?

Erectile dysfunction (ED) is simply the repeated inability to get or to keep an erection firm enough for sexual intercourse.

You might find that you can’t get an erection at all, or you can get one only sometimes or you can only keep an erection for a brief period of time.

Some people believe that ED is just a part of getting older – it’s not. It’s caused by a physical or psychological problem that can be treated. And because it’s such a personal issue, you might feel awkward about seeking help but don’t worry: ED is very, very common and is very treatable. Causes of erectile dysfunction

1. Arterial narrowing caused by:

High blood pressure and high cholesterol
Smoking, overeating and lack of exercise

2. Reduced testosterone levels

3. Psychological factors

Depression, anxiety and stress resulting in infrequent sexual arousal or sexual inactivity

4. Effects of prescription medications (some antidepressants, high blood pressure medicines and antihistamines) but we can work around these.

5. Other causes

- Alcohol and drug abuse
- Spinal injury
- Treatments for prostate cancer
- Diseases that affect the nerves, like multiple sclerosis

Diagnosing erectile dysfunction at the clinic

Your Dublin Medical Centre doctor will look at your medical and sexual history. This will help identify if any previous diseases might have led to ED or if there have been any problems with sexual desire, erection, ejaculation or orgasm.

Some prescription or other drugs might be a chemical cause (drug effects account for 25 per cent of cases). A physical examination can give clues to problems in your nervous system, for example. There might be hormonal or circulatory problems that we can quickly identify and begin to treat.

Sometimes we will do laboratory tests to check for systemic diseases, including blood counts, urinalysis, lipid profile and measurements of creatinine, liver enzymes and free testosterone.

Sometimes we will monitor erections that occur during sleep to help rule out certain psychological causes. Healthy men have 4 or 5 involuntary erections a night during sleep lasting 20 to 40 minutes each.

Treating erectile dysfunction

It’s very important to realise that ED can become worse the longer you leave it untreated. We often find that what starts as a small setback can really affect your quality of life because it plays on your mind which, in turn, further affects erections and you’re caught in a vicious circle.

Treatment can be on a number of levels, depending on the causes and severity of the problem, and is tailored to your specific case. This is why accuracy of diagnosis is so important.

Treating ED can be as simple as making a few healthy lifestyle changes such as quitting smoking, losing excess weight, reducing alcohol or getting more exercise.

The next thing to consider is oral or locally injected medications and possibly hormone replacement such as testosterone.

In very resistant cases, we will consider vacuum devices and surgically implanted devices.

If it becomes clear that the underlying causes might be emotional or relationship problems we will arrange for an excellent counselling referral.
